Probably just didn't know how to tighten it properly. Getting it nice
and tight and in the exact orientation you want can be a bit of a
chore. After all the trial and error adjusting the nuts on the shaft
and still ending up with a crooked knob, I can see someone getting
frustrated and just lubing it all up with contact cement.
