[DML] Hesitation when cold

I know this has been posted before but I couldn't get into the archieves
to search for it. (URL Not Found)

When I first start the car cold it takes several tries before it will
idle. It will catch right away then die. After several attempts it will
grudgingly idle. If I try to drive it the car will die or cough and
backfire. If I let it idle for three or four minutes it runs fine and
accelerates fine. Whats not working in those first couple of minutes????
