It might be as simple as your air filter. My car would pull the "almost die" stutter once a trip if I didn't let it warm up for a minute or so before driving. I'd add gas to compensate for the hesitation and it would be fine for the rest of the trip. Now that I've changed my air filter, which was really cruddy, there is just the barest of hesitations at the old "almost die" point, the car idles better, smells better and has significantly more power.
