[doc] Me Nuts are falling to pieces

Hi List,

Sorry if this obvious to anyone but i took my rear wheel off for the 
first time and the nuts kept breaking apart, specifically there is 
the nut and an outer skin which has separated. The outer is the 
correct size for the socket. Granted I was using an air gun but to 
my defence I was going easy.

Is this usual?
