[doc] Re: DMCH Order Problems

I would *love* to find out why this happens. It seems to happen more
frequently from users outside the US (paricularly in the UK), but
occasionally from within the US, too. I cannot pinpoint any correlation
between OS (other than it always seems to be Windows machines) or browser.
