It sounds like too much fuel during start - probably from the cold-
start valve which is activated during the cranking phase. When you 
hit it right away, without the pause for pressurization, you catch 
the fuel system and cold start valve with the least amount of 
pressure on it and the least amount of fuel entering the engine 
during cranking.

I would disconnect the cold-start valve or the thermo-time switch in 
the thermostat housing to see if the starting behavior improves. If 
it does then one of those two components is faulty.
