If you look carefully, the guys arm doesn't look "big" enough
(perspective-wise) to be at the plane of the windshield.  It looks more
like he's gesturing towards the B-pillar or the seat back, and it's just
coincidence that it lines up with the wiper.

But that would be pretty silly if it were on the inside...
