I did the online test at http://www.imaca.org/ a year or so ago.  It's 
only $15, and requires about 15 minutes of looking at their little online 
guidebook to get a perfect score.

They'll give you certification immediately if you provide them your SSN, 
or a few days later in the mail if you don't want to give them that (the 
route I took).
