Every magazine has an "oops". The better ones admit to it (albiet in small print on an inside page). Since magazines are created by humans, errors get introduced and sometimes some slip through. I never saw any corrections in any of the DW magazines I have read. As for credits, most magazine editors try to give due credit, it is not always easy. Add to that the urgency of deadlines and it is not high on their priority list.
