I found it. As of January 2008, DMCTalk member "91tbrick" had it: http://www.dmctalk.com/showpost.php?p=106983&postcount=40 I will send a PM to see if he or she wants to sell it (I do not have time this fall to make a bunch of manifolds). His/her profile does not list a location, but there's snow in the picture. That adapter appears to be welded and ground, and thus pretty much a permanent part of the manifold (some carb choices might require an additional adapter).
