I would check the resistor on the blower motor. Less likely but possibly worth checking is the circuit breaker in your relay compartment. My fan speeds 3 & 4 quit working intermittently, and it was because the circuit breaker had come loose from its clip and was sitting under the fuse box, thus overheating and turning off intermittently.
