[DML] AC Leak In Passenger Compartment?

Hello all.  
This past weekend I took a trip out of state in my D and later learned 
that I had accumulated a large amount of water under the passenger side 
carpets.  The day was sunny and hot so no moisture got in from the 
underside of the car.  I have to assume this is water from the AC unit.
What are some things that would cause this large amount of water to 
gather unnoticed on the floor boards of the passenger side (only a 
minimal amount of water gathered on the driver side)?  The only thing 
that I can come up with is that the drain hose somehow came loose from 
the AC down spout.  If that's the case, any good ways of fixing the 
problem?  Are there other ways the water could have gathered?
