Sounds to me like your shift computer is malfunctioning. I had a similar problem where I'd always start in second, but I could always get up to third. Unfortunately, it sounds like you can't reach third either. If you're starting in second, I'd say one of two power transistors on the shift computer circuit boards has blown. (which also raises the question of what caused it to blow) As for being unable to shift into third... not sure. Could be a loose solder connection between the shift computer board and the "mini-alternator thingy" that rides alongside the tranny that measures RPM. (had that happen to me too) The signal is bad and the computer thinks you're going slower than you are. I'd definitely say change the tranny fluid and filter first and see what happens.
