Some thoughts - Where does the fuel enter the fuel rail? Have you measured the pressure across the entire fuel rail? I didn't read every response - but I assume you are sure the cam is not flat spotted or a lobe worn down? With a bore scope - how do the valves look at overlap? Are you sure the cam is timed correctly (could it be off a tooth?)
