Both.  They measure emissions at idle and under load.

My CO+CO2 % looks high, but my idle was at 1500 and wouldn't settle
down (just discovered tonight that a wire on the microswitch was
loose)...but the emissions guys calculated in the high RPM and said I
easily passed.
