You are not supposed to run the motor with the pressure tester at 15 psi. The pressure rise is normal, it happens when you heat up the coolant. Take the pressure tester off and see if the cap blows coolant. If the pressure goes above 15 psi when you start out with 0 it does mean a head gasket. There is a test to confirm this called a combustion gas leakage test but you don't really need it. Do not start out with 15 psi and then heat up the motor. Pressure above 15 psi IS bad.
